Peer Learning Group Facilitator Application

Please make sure you have first completed the Peer Learning Group application before submitting the Facilitator application. Additionally, please note, the form will become inactive after a period of time. Applicants are encouraged to write out their statement of interest ahead of time and then copy and paste it into the form before submitting.

The role of facilitator is a key feature of this program. Thus it is important that persons who serve this function have the skill, training, emotional and spiritual stability to function effectively in a leadership role among their peers. Please provide a statement that describes your interest and related experience that will be useful in helping you to fulfill the role of facilitator.


Please provide two references – one from the chair of your church’s leadership team and one from a clergy colleague. References must be emailed directly from the referring individuals to the program, along with the reference form. Reference forms can be downloaded from under the facilitator tab.
